SKIN RESURFACING TODAY

The technology used today replaces the once used ablative resurfacing that saw the plastic surgeon remove the top layer of skin from the patients face leaving the patient to then suffer with oozing, crusting and seepage from the treatment sites for weeks while the entire surface of the skin healed. Differences Between CO2 And Erbium Fractional Laser Treatments

There are 2 types of ablative lasers: iPixel carbon dioxide (CO2) and erbium (also known as ND: YAG) lasers.

Both help resurface the skin and act as an ideal remedy for those whi wish to resurface the skin while lifting and tightening.

What Are Ablative Lasers?

Fractional laser beamscreate tiny microscopic holes in the epidermis to initiate the process of skin remodelling with the production of collagen. Collagen is an essential part of the human body. It produces structure and support for the skin, organs, and other vital components of human biology. Fractionated lasers cause thermal injury in the dermis, and that signals the body to produce more collagen in the damaged areas, which results in controlled tissue vaporization. Over time, this improves the texture, tone, and firmness of the skin’s surface, which are the results that most people want when they consider laser resurfacing treatments. .

Comparison of Erbium and CO2

iPixel YAG – Erbium

– YAG laser has a shorter wavelength that equates to a lower level of penetration into the skin. – YAG lasers have a higher-power density in terms of focusing in on specific areas. – YAG lasers have a small Rayleigh length* vs. CO2, which has a longer Rayleigh length. – YAG is great for quicker processing of thinner materials – Can be Q-switched for pulsed or continuous wave output operation. (This is a feature used in laser ablation.) – Erbium laser provides a milder, less invasive treatment option than a carbon dioxide (CO2) laser. The erbium laser skin-resurfacing treatment is the ideal choice for those who would like to dramatically improve their skin’s appearance. It removes superficial scars and lines in addition to deep wrinkles, and it improves the appearance of the texture and tone of the skin. Fractionated CO2

– CO2 (molecular formula for carbon dioxide) laser has a longer wavelength, which equates to a higher level of penetration into the skin. – CO2 lasers have a high Rayleigh length* vs. YAG lasers, which have a shorter Rayleigh length. – Requires varying focal lengths for different ablative applications – Capability limited to one mode: CW (continuous wave) operation * Rayleigh length refers to the capacity of the laser to withhold energy so that it can process and cut through thicker materials. The higher the Rayleigh length, the higher the ability to cut through thicker materials. Ablative lasers can target varying depths into the epidermis. The procedure is often more intense than other laser treatments due to the puncturing of the skin in micro needle holes, so most patients do require pain relief during their treatments .  CO2 laser treatments are able to address deeper layers of damaged skin by removing more of the skin’s superficial layers. However, with more intense treatments comes a longer recovery period.

About Fractional Skin Resurfacing (CO2 and Erbium)

 

The Treatment

We ask that you come for your treatments with your skin clean and dry without makeup. A pure moisturizer is acceptable but should be free of perfumes and chemicals. In some cases, a topical anesthetic can  be applied for 20 – 40 minutes pre treatment to help numb the treatment site and provide increased comfort for the patient. This anesthetic will be removed prior to the treatment. You will put on protective eyewear for the duration of the treatment.

Treatment Discomfort

 A feeling of warmth will increase as the treatment progresses and after the completion of the treatment the skin will feel as though you have a sun burn. That sun burn feeling will increase over the approx. 2 hours following the treatment, however, if you are consistent in your post treatment cooling efforts, the feeling of heat will dissipate more quickly than if you do not post cool.

Post Treatment

Once the treatment has concluded, you will have cooling cloths, ice, warm water and vinegar washes (for post CO2) applied to the area and a highly hydrating post moisture cream will be applied. The redness will last anywhere from several hours to several days depending on the aggressiveness of the treatment for Erbium and a week to 10 days for the CO2. The more you cool the treated area the shorter you will feel heat and the swifter the redness will disappear. CO2 will have a color change in the skin to brown and then a sloughing process. After several days the skin will flake (not peel) and you may feel like the skin is tight and dry. You will have a specialized, highly hydrating moisture cream to apply that will help with the dryness which can be applied in small amounts for the first several days. Depending on the aggressiveness of your treatment series, you will be able to apply mineral makeup as soon as the sunburn feeling has dissipated and you will be able to resume your normal daily activities immediately upon completion of the treatment however you may want to avoid public exposure until the critical healing phase is resolved. You should avoid direct and prolonged sun exposure and not use highly abrasive cleansers on your face nor should you use creams with heavy perfumes or chemical ingredients for several weeks post treatment.


Number of Treatments

The number of treatments required and the spacing are dependent on several factors including; your age, level of damage and level of health which will determine the ability of your body to regenerate. Usually, for Erbium, 3 -5 treatments are delivered 2-4 weeks apart. For CO2, often one treatment is often sufficient depending on the aggressiveness of the treatment and the amount of downtime acceptable.

Results

The results will be evident within the first week after your treatment series begins and will continue to become more evident as long as 6-12 months post treatment. The length of time the results last are dependent on general health and lifestyle factors.  The fibroblasts deep in the tissues in the skin take time to regenerate and the process of collagen and elastin redevelopment takes time to become fully complete. It is important to remember that we are always aging. The treatments offer prevention, correction and maintenance of your skins integrity and the length of time between treatment series will depend on your skin care regimen once the treatment series is complete. Fractional Radio Frequency Micro Needling

Microneedling is a popular procedure that can safely and effectively address a host of skin concerns. Traditional Microneedling

Traditional microneedling uses a special device that creates micro-injuries in the skin, stimulating the body’s own healing response. Benefits of RF Microneedling

Fractional RF Micro Needling

Photodamaged and aging skin is characterized by the thinning of the epidermis and dermis and is associated with a decrease in collagen levels resulting in fine lines, wrinkles, tone and texture changes, and lax skin. Treatment of photoaged skin is based on inflicting damage to the dermis, resulting in neocollagenesis. Damage to the dermis results in the upregulation of the activity of various enzymes, such as metalloproteinases and collagenase that are responsible for the removal of old tissue and deposition of new dermal tissue1. Traditionally, laser-based technologies are used to resurface the skin. However, these treatments vary in their efficacy, adverse effects and downtime after treatment.

Microdermabrasion is a general term for the application of tiny rough grains to buff away the surface layer of skin. Many different products and treatments use this method, including medical procedures, salon treatments and creams and scrubs that you apply yourself at home. It’s usually done to the face, chest, neck, arms or hands. Before we can understand how microdermabrasion does what it does, it’s important to understand how the skin works.

Your skin is made up of two main layers, the epidermis and the dermis. The epidermis is the layer closest to the outside world. It’s a set of dead skin cells on top of another layer of cells that are in the process of maturing. The topmost layer is called the stratum corneum. The stratum corneum mostly acts as a barrier between the outside world and the lower skin layers. It keeps all but the smallest molecules from getting through.


Microdermabrasion Effects

The principle is that if you remove or break up the stratum corneum, the body interprets that as a mild injury and rushes to replace the lost skin cells with new and healthy ones. In the first hour after treatment, this causes mild edema (swelling) and erythema (redness). Depending on the individual, these side effects can last anywhere from an hour to two days.

This process has a few beneficial effects. With the stratum corneum gone, the skin’s surface is improved. The healing process brings with it newer skin cells that look and feel smoother. Some of the skin’s visible imperfections, like sun damage, blemishes and fine lines, are removed. Also, without the stratum corneum acting as a barrier, medicinal creams and lotions are more effective because more of their active ingredients and moisture can find their way down to the lower layers of skin. As microdermabrasion temporarily removes some moisture from the skin, it is always followed by the application of moisturizing creams.

Early studies suggest that repeated microdermabrasion treatment at regular intervals may influence the way the lower layers of skin grow, as well, removing deeper blemishes over time. Some evidence seems to indicate that the rapid loss of skin moisture may be what triggers the lower skin layers to work overtime in speeding healthy cells up to the surface.

The Treatment

The tool used shoots a stream of tiny crystals over the skin and collects the leftover dead skin cells and used crystals.

The vacuum action of the machine has four main functions:
